- So, you are thinking about adopting and would like to find out more about Forever Bound Adoption. We are excited about the prospect of helping you build your family, and would love the opportunity to answer your questions and help you better understand your options!
- This all starts with a Pre-Screening Adoption Questionnaire. Please complete
this questionnaire, which will help you, and us, determine if adoption with us is a good fit for you. - After completing and submitting the form, if you would like to proceed, you will
need to schedule a virtual meeting with one of our caseworkers. Please call our office to do this at 385-445-4701 - After completing the virtual meeting, if you decide to proceed with Forever

Online Adoption Application Process
NOTE: Fill out this Apply to Adopt Online information form only if you have completed the VIRTUAL meeting with our caseworker and are ready to make payment and begin the adoption process.
STEP 1: Fill out the Apply to Adopt Online information form found on the “Apply to Adopt” page https://www.foreverboundadoption.org/adopt/apply-to-adopt/. Once you submit the form you will be sent a username and password for My Adoption Portal (MAP). (Forever Bound Adoption uses a secure online client portal called My Adoption Portal (MAP), for our adoption applications and for gathering the required documents for your home study and file.) This will create your online account. This is NOT the application, this merely creates your account so that you can begin the application process. Note: If you do not see the email in your inbox, please check your spam folder.
STEP 2: Log into your MAP account and please read and sign the Description of Services document. Once approved, you will be given access to the Adoption Application and other application tasks.
Returning Forever Bound Adoption Families-
If you have completed a previous adoption with Forever Bound Adoption and are planning to adopt with us again, please contact our office manager at 801-821- 1354, or email [email protected]. We will review our previous portal account with you and connect you with the application. This will allow us to make sure that your current files are imported properly.
